 The Okmulgee Memorial Hospital’s Health Information Services Department or medical records department is located at the north entrance of the building next to the cafeteria. The department maintains all the hospital’s records and releases medical records to the public and other medical facilities. The department is open from 8 a.m. to 5 p.m., Monday through Friday. Requests for record copies can be made in person or by mail. An authorization form must be filled out to receive any medical information. NOTE: Oklahoma State Law provides that we can charge $1 for a copy of the first page of any medical record and 50 cents for each additional page, plus postage. Record requests may take three to five business days to fill, and archived record requests may take longer.
Birth Certificates
Okmulgee Memorial Hospital doesn’t provide copies of birth certificates or medical records of births in our facility. At OMH, we can only provide a copy of the mother’s medical record that acknowledges she gave birth in our facility, a birth certificate request form and contact information of birth certificate issuers. A birth certificate request form can be obtained by clicking on the Birth Certificates link on the right toolbar.
