|  | Hospital Billing and Insurance Information
If you have heath insurance, we’ll need a copy of your identification card. Your plan may have special requirements, such as pre-certification for certain tests or procedures. It’s your responsibility to make sure the requirements of your plan have been met. If your plan’s requirements are not followed, you may be held financially responsible for all or part of the bill. Deductibles, co-insurance, and co-payments are also your responsibility. If your insurance can’t be verified or if you don’t assign benefits, you will be expected to pay at the time of service or discharge. If you provide insurance coverage after the services are rendered, you will be responsible for any penalty assigned by insurance if pre-certification is required.
NOTE: All OMH patients should familiarize themselves with the terms of their insurance coverage. This will help you understand the hospital’s billing procedures and charges. Depending on your health coverage plan, you maybe asked to assign benefits from the insurance company directly to the hospital.
Medicare
If you’re on Medicare then, we will need a copy of your Medicare card to verify eligibility and process your Medicare claim. You should be aware that the Medicare program specifically excludes payment for certain items and services such as cosmetic surgery, personal comfort items, and others. Also, as a patient of OMH, it’s your responsibility to pay any deductibles, co-insurance, and co-payments that aren’t covered by Medicare.
Medicaid
If you’re on Medicaid, then we will need a copy of your Medicaid card. Medicaid does have limitations on a number of medical services and items that OMH can offer to patients covered by Medicaid. For a list of limitation or any Medicaid coverage questions go to www.cms.hhs.gov/home/medicaid.asp.
No Insurance
If you don’t have insurance, we have financial counselors available to assist you in making financial arrangements to pay your hospital bill.
