Welcome to Okmulgee Memorial Hospital, we were founded in 1967 when the city recognized the need for a more modern, updated hospital. At OMH, we aspire to provide our patients with the highest quality of care possible in the most efficient manner. We promote a family-oriented atmosphere because we want our patients to receive the care and support only family can offer. We also realize that being admitted to a hospital can sometimes be a traumatic and stressful experience, especially if you don’t know what to expect, which is why we compiled this patient information page.
Admissions
When entering the hospital, patients will first be directed to admissions, which is located in both the ER and Outpatient Services. The ER is located at the west entrance of the hospital and is for patients in need of emergency care. Outpatient Services is located at the north entrance of the hospital and processes all of the hospital’s patients. While in admissions, patients fill-out all necessary paperwork and then are given their hospital identification number.
Things to bring to admissions to ensure quick hospital admission:
- Active Insurance or Medicaid Information. For a list of insurance providers accepted by OMH click here.
- Picture ID
- Social Security Card
- List of Current Medications. For a medication log click here.
- Name and Phone Number of Current Primary Care Physician
- List of Allergies
- List of Emergency Contacts or Next of Kin. For a medical information sheet click here.
- Advance Directives, such as living wills and powers of attorney. For a copy of the Advance Directives click here.
- Funds for paying patient portion of insurance such as, co-pays, deductibles or self-pay portion
Admissions will also ask that patients sign a Patients Rights and Responsibilities consent form. All information patients share with our highly trained admissions representatives become part of their medical records and are kept strictly confidential. The Outpatient Registration office is open 7 a.m. to 5 p.m., Monday to Friday, and the emergency room never closes.

Patient Identification/Information
A patient’s hospital identification number identifies what the patient is in the hospital for, what medications they receive and what room they’re staying in. This is done to ensure the safety of not only the patient but also every member of the hospital, so please don’t remove your wristband while in the care of the hospital. Patients should also bring a list of current medications and emergency contacts. Don’t bring medicines themselves, unless otherwise informed, bring only a list of current medications. Patient Accommodations
After being admitted into the hospital, patients are then taken to their rooms. Depending on the unit, most rooms will have cable TV, phone and wifi access, and 80 percent of the rooms are private. Arrangements can also be made for family members to stay in the room with a patient. Upon request, newspapers will be delivered to a patient’s room.

Nutritional and Dietary Services
Patients receive three meals a day which are served at:
- Breakfast – 8 a.m.
- Lunch – 12 p.m.
- Dinner – 5 p.m.
Snacks are also served to diabetic patients at night or to all patients upon request. All meals and snacks are provided by Nutritional and Dietary Services, which can be reached at ext. 4161. Patients are provided with a new meal and snack menu every day of their stay in the hospital, and can choose from a wide variety of healthy and delicious meals and snacks.

Housekeeping
At OMH, our main concern is of the health and well being of our patients. To ensure patients are being cared for in a clean and friendly environment the hospital employs a specialized staff of highly skilled environmental service workers who sanitize and sterilize the facility. These workers are also responsible for the sanitation and sterilization of patient rooms. Environmental service employees clean patient rooms every day, and can be reached at all times by calling ext. 6504 if further housekeeping duties are needed.

Chapel
Okmulgee Memorial Hospital Chapel is located on the second floor of the hospital. The chapel was created as a place for hospital patients and visitors to meditate, pray and worship. It’s conveniently open 24 hours a day, seven days a week. All chapel furnishing and décor were provided by the generous contributions of OMH Volunteers.

What to Bring
When entering the hospital as a patient, it’s important to know what is appropriate to bring on hospital premises. Don’t bring tobacco products on hospital premises; OMH is a smoke-free facility, meaning that no smoking will be tolerated in the hospital. Patients, visitors and all hospital employees and volunteers are asked to only smoke in designated smoking areas, while on hospital premises. Patients are discouraged from bringing jewelry, credit cards, medications, electronics, appliances, large sums of money and other valuables from home. It’s OK to bring small amounts of cash for discretionary spending. Do bring toiletries, a robe, sleepwear, slippers and personal items, such as eyeglasses and denture cases. Patient Concerns
At OMH, it’s our goal to ensure that our patients receive the best quality of care possible. We understand; however, that sometimes problems can arise. When this happens, we encourage the patient or family member to express their concerns and complaints to the hospital staff. Often times, concerns and grievances can be settled at the time and place of service, but if you feel the issue was left unresolved, then the patient or family member can make a request to discuss the issue with the department supervisor.
The department supervisor will then investigate the complaint, within two working days, and make an effort to resolve the matter. After the department supervisor has investigated the issue fully, the complainant will receive a written notice of the investigation and planned resolution of the problem within 10 working days from the original day of the complaint.
If complainants still aren’t satisfied with the response by those involved or by the department manager, they can fill-out a patient grievance form, which is available through the admissions, quality improvement and administration departments.
OMH will make every effort to resolve all patient grievances, but if a patient is still not satisfied with how we handled the issue they can at anytime contact Oklahoma State University Department of Health Peer Review Organization at 1.800.522.3414.
OMH also does follow-ups with our patients to gain insight on patient care. We may contact you, and if we do please take a moment to respond to our survey questions. Your comments will help us improve the way we provide care and to recognize the efforts of our staff.

Patient Visiting Hours
- Medical/Surgical Unit, 2nd floor – 9 a.m. to 9 p.m.
- HOPE Unit – 4 p.m. to 6 p.m. Monday through Friday and 12 p.m. to 6 p.m. Saturday through Sunday NOTE: Visitors must receive prior approval to visit patients in the HOPE Unit.
