Okmulgee Memorial Hospital Social Services is located on the second floor of the hospital. The OMH social worker provides information and referral services to both hospital and home health patients. The social worker’s responsible for providing discharge planning to hospitalized patients, which is the assistance of patients in allocating appropriate after-hospital care. Discharge or care planning, involves the social worker working closely with the patient’s physician and, or care team. The social worker also provides assistance to people inquiring from the community for guidance with health care issues.
The social worker empowers patients and their families by providing them with the education and information needed to make informed decisions about their health care options. The social worker is also an advocate for individuals who are unable to advocate for themselves. Questions most frequently addressed by the social worker are health care questions related to financial benefits and resources, but he also answers questions about criteria and processes to access services and help.
The social worker is here to provide families with support and guidance when making decisions regarding keeping frail elderly family members in the home safely or placing them in a 24-hour-care facility. Another important service offered through the OMH social services department is assistance in completing end of life forms, such as advanced directives and living wills.
